Referrals and Admissions

Referrals for Hospice admission are accepted from a variety of sources including GPs, hospital consultants, community nurses, Macmillan nurses, hospital and community AHP's. In general, referrals are accepted for patients resident within the boundary of Oldham and its surrounding districts although a small percentage of admissions are from outside the immediate area. The population served by the Hospice is around 235,000 people.

The Hospice operates a planned admission policy. The clinical palliative care Multi-Disciplinary Team (MDT) have input into the planning of patient admissions and assist in assessing priorities. A similar admission policy operates for the Wellbeing Centre.

Patients may be admitted at any stage during their illness for appropriate treatment and support. Admission may be for symptom control, or care during the terminal stage of illness.
